Blooming Access

End-of-life support should feel accessible when families need it most.

Blooming Access is a limited community support program through Blooming Transitions that provides a small number of complimentary or reduced-cost end-of-life doula and caregiver support hours to individuals and families experiencing financial barriers.

Life at the end of life can already feel overwhelming. Asking for support shouldn’t have to make it harder.

What Blooming Access Can Provide

Approved recipients may receive up to 4 hours of support, based on current availability and individual needs.

Support may include:

  • End-of-life education and guidance

  • Caregiver support and respite

  • Companion visits

  • Vigil support

  • Anticipatory grief support

  • Advance planning conversations

  • Legacy support

  • Virtual guidance and family support

  • Help understanding and preparing for changes during the dying process

Blooming Transitions provides non-medical support and does not replace hospice, nursing, medical care, counseling, or emergency services.

Who Can Apply?

Blooming Access is intended for individuals and families who would benefit from end-of-life or caregiver support but may not currently be able to afford services at the standard rate.

You do not have to be enrolled in hospice to apply.

Applications may be submitted by the person needing support, a family member or caregiver, or someone assisting the family.

Because Blooming Access is a limited program, submitting an application does not guarantee services. Availability is based on current capacity, location, level of need, and available program resources.

How It Works

1. Submit a short application.
Tell me a little about your situation, what type of support would be most helpful, and any scheduling needs.

2. I’ll review your request.
Applications are reviewed individually with care and discretion.

3. We’ll connect.
If Blooming Access is available and your needs are within the scope of Blooming Transitions, I’ll reach out to discuss the next steps and determine how the available hours can best support you or your family.

A Little Support Can Make a Difference

Sometimes a family needs someone to sit with their loved one so they can rest. Sometimes they need help understanding what they are seeing. Sometimes they need someone to help them prepare for what comes next. And sometimes they simply need to know they do not have to figure everything out alone.

Blooming Access was created to make room for those moments.
